Forms

Prior to participation in any TPS athletics, several forms must be completed. 

PHYSICAL HISTORY AND EXAM

All swimmers need to turn in the two sports physical forms which you can download as PDF documents below: Preparticipation Physical History and Preparticipation Physical Examination. These forms should be printed and returned to Coach Norman before swimmers can participate in summer practice. The History form should be filled out and signed by the swimmer and a parent/guardian; the Physical Exam form must be completed and signed by a physician.

TPS Athletics/RankOne Sport Online Particpation Forms

All TPS student athletes/parents/guardians must complete the TPS Athletics Online Particpation Forms before being eligible for participation in their respective sport.  Everything you need to know about filling out the forms is on the Instructions page you'll find at the TPS Athletics/Rank One Sport site linked in orange below. Read the Instructions page first and gather all the information it says you'll need to fill out the online forms. This will make filling out the forms much easier and faster. It takes around 15 minutes to complete all four forms. Please note that these online forms are in addition to the paper Physical Exam and Physical History forms you should also required by TPS Athletics.

All four forms must be signed by a parent/guardian; three of the four online forms must be signed by the swimmer and a parent. Have your student there with you to sign along with you electronically. You use your computer mouse or trackpad to sign the forms electronically. 

To start, go to Tulsa Public Schools Online Particpation Forms powered by RankOneSport - this website will open in a new tab. 

Please note that you can now register on the RankOneSport site to access your student athlete's info more easily. This is separate from any other TPS website registration. If you click on the "Log In" icon atthe top left of the page or the "Start Online Forms" icon at the bottom of the Instructions page, you should be taken to a page where you can register (or log in if you've already registered) on the Tulsa Public Schools Online Participation Forms page. Once you're logged in, click on Add Child if your student's name is not already on there. Enter your child's last name and Student ID number. If you have more than one student athlete, you can add another child. Once you've added your students, you can access their info to see the status of your online form submissions. You are not required to register on that page to fill out the forms. You can fill out the forms without registering.  

If you do register on the site, complete the registration then log in and proceed as follows:

  • Read the Instructions page and gather all the info you might need to complete the forms. (This page also includes information about submitting the hard copies of the Physical History and Exam forms which everyone should have turned in by now.)
  • Click on the Fill Out Forms tab, click on your swimmer's name. 
  • Fill out the first four forms listed on this page and sign them electronically: 
    • Emergency Information Form (to be signed by parent only)
    • OSSAA Eligilibility Form (to be signed by student and parent)
    • OSSAA Rule 9 Recruiting Form (to be signed by student and parent)
    • Concussion & Consent Forms (to be signed by student and parent)

If for some reason you do not want to register on the site, then proceed as follows: 

  • Read the Instructions page and gather all the info you might need to complete the forms. (This page also includes information about submitting the separate hard copies of the Physical History and Exam forms which everyone should have turned in by now.) 
  • Click on the Electronic Participation Forms tab at the top. 
  • Fill out the first four forms listed on this page and sign them electronically: 
    • Emergency Information Form (to be signed by parent only)
    • OSSAA Eligilibility Form (to be signed by student and parent)
    • OSSAA Rule 9 Recruiting Form (to be signed by student and parent)
    • Concussion & Consent Forms (to be signed by student and parent)
