PHYSICAL EXAM AND PHYSICAL HISTORY INFO & FORMS
Swimmers will not be able to participate in swimming until they have turned in a Preparticipation Physical Exam form filled out by a physician and a Preparticipation Physical History which is filled out by swimmer/parent.
These two forms are available to download as PDFs at the bottom of this page. You'll need to print these out.
Swimmers need to turn the physical exam and history forms in to Coach Meilner on the first day they come to summer swim. They can not use last year's physical exam. A new physical exam must be done by a physician and it must take place after May 1st each year.
You can schedule a sports physical with your regular health care provider or use one of the OU Physicians centers listed below since they can sometimes see patients sooner for these specialty physicals. Since appointments cannot always be scheduled quickly, please call for an appointment ASAP! Please note that most health insurance does not cover sports physicals.

What to bring to the doctor's office: Printed copies of the TPS Preparticipation Physical History form and Prepaprticipation Physical Exam form (both must be signed by a parent.)
