BTW Swim - Please Fill Out TPS Athletics Online Forms ASAP

2019-2020 BTW Swimmers and Families,
The TPS Athletic Department requires all swimmers to fill out the TPS Athletics Participation Online Forms. These forms must be completed by the student athletes and parents. Please complete these forms as soon as possible - this weekend, or as soon as you can next week. Swimmers will not be able to participate on the team when school starts if these forms are not submitted.
Go to the TPS Athletics Online Forms page on our team website and read the instructions there to begin the process. It takes about 15-20 minutes to complete the forms and you need to have your athlete there with you to answer questions, read guidelines, and sign the forms electronically with you.
You'll see that it lists "Paper Documents to Be Submitted: Physical - Incomplete" at the top of your student's info. This refers to the paper physical history forms that you should have turned in to the coaches at the beginning of the summer. These have been given to the BTW Athletic Director and he has to enter those manually. As long as you have turned in your paper physical form to the coaches, you don't need to do anything else about that. Once that is entered by the AD, it should show as “Approved.”
